The STP (Sustainability and Transformation Programme) is the local roadmap that enables us to deliver NHS England’s 2014 Five Year Forward View, which, sets out a vision for an improved NHS, how we can achieve this, and how everyone involved needs to work together to improve health and care. CPFT is responsible for just one of 44 STPs in development across the country tasked with making the NHS fit for purpose in a fast-changing health economy.Over the next five years, pressures on the NHS will be growing faster than funding. That means every pound has to be spent as efficiently as possible. This requires change to ensure services remain sustainable and of the best possible quality.Cambridgeshire and Peterborough is a diverse region; an area with a population of almost a million people. It has urban areas that have higher levels of poor health and a large concentration of hospital services, alongside towns and rural areas that have different challenges, including physical access to services.The cost of non-actionIf the local NHS does nothing, we estimate there will be a financial shortfall of £250million by 20/21 in Cambridgeshire and Peterborough alone; that is clearly unsustainable. By working more collaboratively and using joint resources, we believe we can address this gap and break even. We know that by working more cooperatively across organisations we can reduce replication of functions, waste and unnecessary expenditure, while providing a better health experience for patients. A key element of our plan is about improving the health and well-being of our population, thereby reducing reliance on NHS and care services.How do we do it?In order to use resources more efficiently, much more focus is needed on preventative care; working together to find innovative ways of meeting the needs of both patients and clinicians and identifying ways of doing things more effectively and efficiently. Overall, all partners – all of us – need to work together to improve health, finances and quality of care to meet the future needs of patients.Cambridgeshire and Peterborough’s STP has been developed in collaboration with clinicians across the region. The STP reflects the support clinicians have shown for new models of care that improve quality, reduce disparity and deliver more efficient care. The options for change discussed in the STP are also designed to reduce replication and waste in the delivery of our core services and embrace new, pioneering practices to improve the quality of care patients receive. Reduced spend in non-frontline functions will enable additional spend and effort to be directed towards frontline services. Improved integration of health and social care means closer working in areas like community services – in our particular case, those who are elderly and/or frail – and mental health. The STP will create more opportunities for earlier patient intervention, leading to healthier communities and less reliance on NHS and care services.
